Payroll Forms

As part of the onboarding process with the Be In Christ, employers must submit payroll forms. Below are the payroll forms for onboarding, concluding and making changes to employee payroll accounts. Please contact Payroll Services if you have any questions about how to use these forms and they will be happy to help you. The forms below are online submissions. Once completed, they are automatically sent to payroll services.

New employees are required to submit each of the two forms listed below and banking deposit information to Payroll Services for processing.

  • Include a void cheque or direct deposit form as issued by your bank.
